Lakeland Care District
Welcome!
The Lakeland Care District is the name of the new regional long term care district we've created to serve qualified elderly individuals and people with disabilities living in Fond du Lac, Manitowoc and Winnebago counties.
Our goals include ensuring that consumers throughout our region have access to high quality services and eliminating waiting lists so those who need services receive them. We will support our members in their choice to remain independent at home and in their community for as long as possible.
In developing this district, our planning is centered on member
choice, quality care and services, increased access, cost-efficiency
and integrated care. We’re building from the experience of our
county long-term care staff and other local programs, as well as
similar initiatives across Wisconsin. The Lakeland Care
District is being designed in collaboration with our local partners
including consumers, providers, advocates and officials.
We hope you’ll use this site to learn more about and participate in
our efforts to expand managed long-term care throughout Manitowoc,
Winnebago and Fond du Lac counties.
