Lakeland Care District
Welcome!
The Lakeland Care District (LCD) is a regional long term care
district providing Family Care program services to
qualified elderly individuals and people with disabilities living in
Fond du Lac, Manitowoc and Winnebago counties.
Our mission is to: Enrich
members' lives by honoring their values through high quality, cost
effective long-term care.
The Lakeland Care District focuses on member choice, quality
care and services, increased access, cost-efficiency and integrated
care.
To enroll in the Lakeland Care District you must be determined eligible by the County Aging & Disability Resource Center (ADRC). Seniors and adults with either physical or developmental disabilities may be eligible for Family Care participation. Participation in Family Care is voluntary.
